This PART II must be completed by approved grantees WITHIN one (1) month after the completion date noted in your Part I Grant Application.
If this Part II has not been timely submitted, grantee will forfeit their 2nd 50% grant payment. If your event/project/program did not go forward, ACPC may request the return of your 1st 50% grant payment.
